What Is YAG Laser Treatment?

Understanding Narrow Angles and Glaucoma Risk: How Laser Iridotomy Helps

The drainage angle of the eye — the channel through which aqueous fluid flows out of the anterior chamber — can be anatomically narrow in certain individuals. This is more common in Asian populations, including Singaporeans, than in Western populations, partly due to differences in eye anatomy.

A narrow angle raises the risk of acute angle-closure glaucoma: a sudden and painful spike in intraocular pressure that can cause permanent vision loss within hours if not treated.

Laser Peripheral Iridotomy (LPI): Preventive Treatment

YAG laser peripheral iridotomy (LPI) creates a tiny, microscopic opening in the peripheral iris using the Nd:YAG laser. This opening allows aqueous fluid to flow freely between the posterior and anterior chambers of the eye, equalising the pressure and preventing the angle from closing. The procedure is preventive and takes approximately 10 minutes per eye.

Consultation and Diagnosis

Our eye specialist performs a slit-lamp examination to assess the posterior capsule, confirm the extent of opacification, check visual acuity and measure intraocular pressure before proceeding.

Mydriatic Drops Applied

Dilating drops are instilled to widen the pupil, providing clear access to the optical axis and posterior capsule. You will wait 15 to 20 minutes for full dilation in the clinic.

A local anaesthetic is applied as eye drops to the eye surface. There are no injections and no discomfort during the laser itself.

YAG Laser Treatment (under 5 minutes)

You are seated at the laser machine and asked to look straight ahead. Post-Procedure Assessment

Intraocular pressure is measured again immediately after to check for any pressure rise. Anti-inflammatory eye drops are prescribed for a one-week course.

What to Expect and Recovery

During the Procedure

  • You will see brief, rapid flashes of light. Not painful — just momentarily bright
  • No sensation of cutting or heat. The laser disruption occurs internally without you feeling anything
  • Vision may appear slightly blurred immediately after due to dilating drops and floating debris from the disrupted capsule. This clears within a few hours
  • Mild eye irritation may occur after the anaesthetic wears off. This is managed with prescribed anti-inflammatory drops

Recovery and Aftercare – Same Day, Zero Downtime

  • Return to normal daily activities the same day
  • Vision continues to improve over the following 24 to 48 hours as the debris from the disrupted capsule settles and clears
  • Arrange transport for the day. Mydriatic drops will temporarily blur your vision and make you more light-sensitive for several hours; driving is not recommended on the day itself
  • Use prescribed anti-inflammatory drops for the full one-week course as directed
  • The results are permanent. Once the posterior capsule has been opened, PCO cannot recur. There is no need for repeat treatment

No. Cataract surgery is a separate, earlier procedure that removes the cloudy natural lens and replaces it with a clear artificial intraocular lens (IOL). YAG laser capsulotomy is a 5-minute outpatient laser procedure performed months or years later, specifically to treat the cloudiness that can develop on the lens capsule behind the IOL. If your vision has become cloudy again after successful cataract surgery, YAG laser — not repeat cataract surgery — is the solution.
No. Only topical anaesthetic eye drops are used. There are no injections, no incisions and no physical contact with the eye. During the laser itself, patients typically see brief flashes of light and feel nothing. Mild irritation may occur once the anaesthetic drops wear off, which is managed with the prescribed anti-inflammatory drops.
Most patients notice significantly clearer vision within a few hours of the procedure as the debris from the disrupted capsule settles. We recommend arranging transport for the day of the procedure. The mydriatic drops used to prepare the eye will temporarily blur your vision and make you sensitive to bright light for several hours. Risks and Side Effects

YAG laser capsulotomy is a very safe, well-established outpatient procedure with a low complication rate. Possible side effects include a temporary rise in intraocular pressure immediately after (monitored in clinic and managed with pressure-lowering drops if needed); new floaters in the days following treatment as capsule debris settles; and in rare cases, a small risk of retinal tear or retinal detachment. Dr Khng will discuss your individual risk assessment at the consultation, and the one-month follow-up includes a retinal check.
YAG laser capsulotomy has a success rate of over 95%, making it one of the most reliably effective procedures in ophthalmology. The vast majority of patients notice a significant improvement in visual clarity within 24 to 48 hours and the result is permanent.
Serious complications are rare. In a small number of cases, a temporary spike in intraocular pressure occurs immediately after the laser and is managed with drops in the clinic. A very small risk of retinal tear exists, which is why Dr Khng includes a retinal check at the one-month follow-up appointment.

For any patient experiencing hazy or glare-affected vision after cataract surgery, it is the definitive treatment and the only way to permanently restore the clarity achieved after the original operation.
YAG laser capsulotomy only needs to be done once per eye because it creates a permanent opening in the posterior capsule that cannot cloud over again. There is no such thing as a repeat YAG procedure for the same eye once the capsule has been successfully treated.
Up to 20% of patients who have had cataract surgery will develop posterior capsule opacification within two to five years of their operation. It is the most common complication of cataract surgery and is entirely treatable with a single YAG laser session at EyeWise Vision.
Most patients do not need a new glasses prescription because the procedure clears the clouded capsule without altering the focusing power of the eye.
